Our Core Programs
Housing Navigation
Our service begins with clients meeting a Housing Navigator, who assists in securing permanent housing.
Following move-in, clients have periodic check-ins to ensure they're maintaining their housing and following lease terms.
The Housing Navigator also mediates communication between clients and landlords to support housing stability.
Enhanced Care Management (ECM)
Enhanced Care Management (ECM) offers personalized, one-on-one support to assist individuals and families, all at no extra cost to the client, through:
- Managing their health
- Accessing vital services
- Navigating complex medical needs
Community Supports
Our goal is to help individuals and families find, secure, and maintain stable housing while connecting them to vital resources for long-term success.
